Have users type the current captcha in backwards. That'll confuse them for a while, and easy to implement.

You know how this stops? Drop to the IRC channel, say you want to help, remove spam. Help deal with this. It's not that difficult, at all. Have 3, 4 of you do it, spam would get eliminated almost as soon as it hits the website. But no, it's always better to complain about every single little thing you don't like. Well, that's the reason spammers win.

There are a multitude of imageboard softwares that have post reports. I'm not sure why we continue to use Futallaby, a software that was just a direct translation of futaba. Even futaba has post reports.
Futallaby doesn't even have bans built in, literally just post deletion. Back in like 2007 I actually wrote a lot of features into the script to make it function more like 4chan (post reports, post cites, moderator accounts, MD5 image hash banning etc) but I don't have the script anymore, and the one site that used it died in 2010. That was back when I had only programmed for about a year, so if you just try, you could probably bring some features into it fairly easily.
Regardless, Futallaby has several MAJOR exploits that were never patched because the maintainer, thatdog, gave up on the software in 2004 (likely because he knew how shit futaba's spaghetti code is). It's also the reason his site, 1chan, runs on wakaba, and not futallaby

All in all, I'd just recommend switching to one of the scripts that has it built in, like tinyboard. If you want to stay on a futallaby based system, you can even use tinyib, which has bannings and reports if I remember correctly. It's been years since I was interested in image board scripts.
